one of the issues with the Harris campaign does seem to be they haven't managed to separate themselves from the unpopular Biden administration and make their case for 'change' from a Dem president. Trump is referring to Harris as the incumbent, which makes it easy for him to play the vote for change card and blame her for anything that has happened in the last four years.

Its pretty obvious from the context that Biden was referring to the comedian. It's different to the deplorables thing because one of Trump's supporters literally has just called Puerto Rico garbage, and they've been threatening to lock up not just Democratic politicians but claiming they'll punish the supporters as well.
You're giving this 'garbage' lot a bit too much credit to differentiate Harris from Biden. The Dems have gone into damage control mode but I don't think it'll work. It's literally a few thousand votes in some of those states who were wavering. It was a fucking stupid thing to do/say. Even if we all know it's true. He's an absolute idiot, in summary.
He's stolen all the headlines on the major networks and papers... a little bit of revenge maybe for bring relegated? He's either very stupid or very petty

The ageing process leads to slips of the tongue and senior moments. Whatever you think about his politics (not a huge fan myself) I think it was a genuine mishap.
He should have just done the one term and not ran for a second as originally promised. But "sedating" him might not necessarily be the answer.
