My first thoughts when seeing the picture was that Kate looked so much younger - worry lines were gone. I put it down to her being relieved that she had a diagnosis, was operated on and now is recovering.

I really can't get my head around why they would they would send it out.

wrt the youngest fingers, this is not the first time people noticed his hand was weird.
