It’s probably a consequence of using a digital camera which is smarter than its owner, and the photo may well have been taken recently with all four parties present, but theories about the Mountbatten-Windsors having dysfunctional marriages while seeking to deceive a press corps that they loathe don’t automatically suggest that they are all-powerful. Just rather entitled and thick.

First thing I thought of too, not any dodgy photoshop. There is no way she could be sitting comfortably in those jeans after major abdominal surgery. Although I guess they could be undone to give her colostomy bag some room...

Occam's razor and all that. The most likely reason I can think of is that several photos all taken a the same time have been merged (probably automatically as is the style these days), but not by a professional or anyone who double checked everything. Why would they, because there is probably nothing to hide. If there was something to hide I would expect them to try a lot harder. It is still quite right for AP et el to drop the photo because you can't be letting shoddy shit like this or AI become the norm in a picture library.
Of course I could be wrong, but I see no reason to currently doubt the most plausible explanation. Not that it shouldn't be questioned. . . and responded to.
