Jakey update: He's still wobbly after the gabapentin he had at 8pm last night.
I phoned the vet and he said start again tonight but just give him half a pill instead of a whole pill at 12 hourly intervals thereafter.
There are little patches of wee everywhere so he's not blocked.
Poor fucking love, he's never been that scrupulous about going in the tray, but this must be doing his head in my poor poor sweetheart.
Weeing everywhere in dribbles and not able to stand up due to the drugs and probably in pain too.
I hope he gets over this quickly (at all tbh, it all seems a bit horrific for the poor lad and if it doesn't clear up pronto or is likely to be a regularly repeated thing I'll have to consider his quality of life given his age and health conditions).

I'm lucky in that I've never had a UTI, but I understand it can be horrifically painful and disorienting.
So it makes sense that he's on painkillers that make him go a bit doolally
But also his kidneys can't clear out the drugs efficiently, so it's kind of like that might actually finish him off anyway, but can't leave him in pain, ugh.
So yeah just keeping everything crossed that the cure doesn't actually kill him and that he can get better.

The vet seemed to think he had a good chance and I'm just going off that basically.
I hope it's not an error of judgement though, because if it turns out his last few days are like this I'm going to find it hard to forgive myself.

You can only do what you believe is right at the time. Trust your gut because you know Jakey better than anyone. It sounds like you've also got a good vet whose professional opinion you can trust too.

Probably doesn't come as a surprise that I tend to overthink things when they are ill.

Have to at least try the gabapentin even if it makes him fall over for hours and might finish him off - it's the first line of treatment, often with pain relief it clears up within 48 hours, and although no pain medication is completely safe for CKD/CRF cats, it is our best option.

Paddy is back from the vet's - minus six teeth :( and a bit disorientated. Much later than we thought, as they'd had a couple of emergencies.

He's obviously hungry, but keeps sniffing the soft and mashed wet food we've given him. He keeps looking at the cupboard with the biscuits in, but he doesn't know he's not allowed them for two weeks!

Did the vet say to mash his wet? Mine always said not to as it's more likely to get stuck in the gums.
Hope he's feeling better in the morning.
 
That's actually very useful to know, thanks! I will take away the pate stuff and leave the ordinary wet food.

Might be worth giving them a ring and checking tomorrow if no specific feeding instructions were given - it may depend on the amount of work done, type of holes left, type of stitches etc.
I've variously (and based on different veterinary instructions at different times) given only dry food, only wet food, and only shredded cooked chicken after different types of dental surgery, so I'm not sure advice from one situation necessarily carries over that well.

I'm glad to hear he's getting back to normal, I've always found they seem to spring back really well after dental work (the main worry is always that it can be long surgery that they have to be out for) - there's often a palpable sense of relief from them - even if you didn't know anything was wrong beforehand.
If only they could tell us if something was hurting, eh?
Go on Paddy, get plenty of grub in you and wake up feeling like a new cat in the morning :)

He was on 50mg which is a fairly high dose for his body weight (only 3.6kg alas, this is a cat that was 5.5kg in his prime!), and I just don't think his kidneys were able to clear that out of his system within the expected 12 hours before his next dose.
I think 25mg will provide the appropriate pain relief and mild sedation while he is still able to walk and hopefully his kidneys won't take 24 hours to get it out of him - I think I was right in not giving him another full dose this morning given that he was still staggering around.
And if need be it is easier to increase a dose than reverse an overdose.

Seemed like it was going well this morning, couple of big wees near the litter tray.
This afternoon it's all gone a bit south, he was back and forth to the litter tray and now has given up and is just sitting on me weeing on my lap so we're both sitting covered in his wee (I really don't care about being weed on per se but him being in this state is heartbreaking). He's also been passing some bloody mucus.
Phoned the vet again and have an appointment tomorrow afternoon, am still trying to get a urine sample, it's difficult because he's just doing little wees everywhere.
