Re accessible rooms in hotels - these may not actually help. The bed is usually set very low, in fact it's so low I find it makes my back problem worse. The room tends to be quite spacious to accommodate a wheelchair (awesome) but the bathrooms are a bit hiit and miss (the showers in particular).
Having an accessible room is not some sort of magic bullet.
Also, first class seats aren't too bad, at least for my back issue.
Flying might make things worse due to the amount of lifting and carrying involved. For a train journey of 4-5 hours, factoring in the travel at both ends and the plane departure/arrival times, plus security etc, there's not a lot in it.
