Probably when I go to Australia I’ll add spare knickers, socks and my toothbrush and toothpaste.
 
Disability seats with an ergonomically designed funnel and tube arrangement for urination, and a privacy hood folding out from the seat in front for when the apparatus is in use.

Until these get installed, and I think they would be a boon for the able bodied as well, I’m with BA Baracus on whether any fool is going to get me on board any plane.
 
Zolpidem/Ambien (I know some people find it trippy and sleepwalk, but for me it's like being nailed into a coffin. In a good way.) Obviously not for a London->Dublin flight though.

Have you been knocked out as often as ba baracus?

Earbuds (short haul) /noise cancelling cans (long haul)
Enough meds to see me through the trip or a week which ever is the shortest (anywhere I think replacement would be easy/enough meds to see me through the trip + a week (everywhere else)
Technical shirt, pants, socks (if bit hand baggage only enough to survive lost luggage for a day or 2)
If I'm not flying in boat shoes I'll sometimes take hotel style slippers
Bottle of water (normally my sports bottle filled air side)

When I'm flying long haul I try and fly with 2 hand luggage bags one of which is pretty small and only has the stuff I immediately need for the flight and I used to be able to fit most of what I wanted in the case for my big headphones.
 
Snacks.
Water.
Earbuds.
Phone with downloaded tv series on.
Big pashmina/scarf.
Lip balm.
Sucky sweets.
Tissues.
Hand cream.
Book.
Painkillers.

It's got a lot less now I don't have young children.
